Authorities are investigating a two-vehicle accident in Buchanan County that killed one person and sent another to the hospital Monday afternoon (May 23, 2016).

The Iowa State Patrol identified the victim as Donald Toale, 60, of Independence. Injured in the crash was Darlene Mitchell, 58, of Oelwein.

According to state troopers, Toale was killed when he failed to yield at a stop sign. His eastbound minivan broad-sided Mitchell's northbound Jeep Wrangler and both vehicles ended up in a ditch.

The accident occurred at the rural intersection of County Road V-71 and 240th St., southwest of Independence, shortly after 1 p.m. Monday.

Authorities said Toale was pronounced dead at the scene. Mitchell was transported to Covenant Medical Center in Waterloo to be treated for her injuries.
